BHRC aims to improve the overall health of the community by working with people in need to access community resources. BHRC’s Charity Tracker Tool assists faith-based organizations, non-profit organizations, and agencies to coordinate with each other to improve services and outreach by providing a comprehensive list of resources and services that are available throughout the Brazos Valley Area.

The BVAAA provides comprehensive and coordinated services that promote the dignity, independence, and quality of life for adults over age 60 in the Brazos Valley Region. Their website includes a list of senior centers and available services including care coordination and caregiver services. Benefits Counseling is available to discuss Medicare Part D coverage, Health Insurance, Medical Entitlements and more.

The Brazos Valley Care Coordination program (BVCCP) is on of the many activities under the Medicaid 1115 Waiver initiative. The program aim to decrease the number of frequent emergency department (ED) users by ensuring appropriate follow-up care upon discharge form the hospital, and connecting these patients to a regular source of primary care and supportive health services. The program connects frequent ED users who do not have a regular primary care provider to care coordinators who will work to provide options for accessing regular primary care and health resources to achieve and maintain a positive health status

The program can help clients with:

  • Obtaining basic health information
  • Learning how to obtain medical services in the community
  • Finding a clinic or medical home to manage their condition(s)
  • Communicating with their healthcare team
  • Finding community resources to help them stay healthy, including needed medications
  • Reminding clients of their appointments and helping to find transportation to appointments

The program has assisted several hundred individuals of the Brazos Valley; and in doing so has decreased the number of inappropriate ED visits by approximately 30% while improving the health of the Community.

The goal of the Brazos Valley Community Action Programs is to assist in the empowerment of individuals, families and communities to reach their full potential economically, educationally, health-wise, culturally, and socially. 

Programs involved include Affordable Housing, Meals on Wheels, WIC, and Utility Assistance.

The County Indigent health Care Program was established by the Texas legislature in 1985. All Texas Counties are to set aside tax dollars to provide medical assistance for their indigent population. To qualify for the Indigent Health Care Program an applicant must provide documentation to verify residence of the County with intent to stay, income, resources and size of family. medical coverage provided by this Program is Physician services, three prescriptions per month, lab work, x-rays, hospital outpatient services and hospital in-patient services. Other medical service maybe provided after physician referrals are provided.

If you are in need of food, there is help available throughout the Brazos Valley from our network of partner agencies. When you visit a food pantry, you will need to bring proof of where you live (driver's license, utility bill, water bill, lease agreement, etc.). You are not required to bring proof of income or citizenship. At each pantry visit, you will be asked to complete a short intake form which asks basic personal and demographic information which will be kept private and confidential. After that, you can visit that pantry once a month to receive food.

The Rapid Re-Housing program strives to achieve:

  • Shorter Shelter stays
  • Housing for those on the streets, sleeping in their cars or in parks
  • Short-term rental assistance to get those chronically homeless, back on their feet
  • Provides housing assistance to Veterans families
  • Provides housing to victims of domestic violence
  • Case management, food, eye/dental assistance, education assistance and work attire is available for those in the Rapid Re-Housing program.
