Falls and chronic conditions are interrelated with similar roots and major risk factors (biological, behavioral, and environmental factors). Therefore, rather than having multiple coalitions for different purposes (fall prevention, disease self-management), the Texas Active for Life® Coalition (TALC) serves as an umbrella Coalition to engage diverse stakeholders to promote healthful aging through collective action.
As seen in the image below, the TALC has 6 primary focal areas:
- Infrastructure
- Funding
- Outreach
- Programming
- Research & Evaluation
- Practice & Policy Change
Through integrated sets of activities and initiatives, coordinated among diverse partners and organizations, the TALC strives to impact the following:
- Scalable & Sustainable Outcomes
- Integrated & Coordinated Services
- Coalition Leveraging
- Aging Workforce Development
- Population Health & Aging
